Consent for Care

Dows Diets will collect relevant social, medical and demographic details to inform your assessment and treatment plan.

You consent for Dows Diets to contact other health professionals involved in your care to gain relevant health information where necessary, or to feedback progress on your treatment where this would be beneficial for your personal care. This will always be discussed with you beforehand.

Participation in a treatment plan

Dows diets will provide you with a dietary management plan that will help you to achieve your lifestyle goals discussed and recorded in your care plan.

All advice given will be evidence based and tailored to individual circumstances.  Clients are encouraged to ask questions and view the evidence if they wish.

Dows Diets will discuss with clients how best to incorporate changes into their own lifestyles. It is your responsibility as a client to incorporate lifestyle changes negotiated in partnership with Dows Diets. Clients are encouraged to take responsibility for their own decisions and actions.

Payment

Payment is accepted by cash or cheque at the time of the appointment. Additional charges will be made for travel at 40p per mile outwith a 20 mile radius of Turriff, Aberdeenshire.

Where payment by a 3rd party, eg health insurer, payment terms are 30 days from invoice date.

Cancellation policy

Cancellations by you on the day of appointments will incur a nominal fee of £5 to cover admin costs.

Cancellation by Dows diets will only be made in exceptional circumstances and a replacement appointment made as soon as possible.

You will be asked to sign a copy of these terms and conditions at your first appointment.
